clinical excellence. We are currently seeking a full-time Hospice
Case Manager (RN) to join our growing interdisciplinary team. If

Overview The Hospice Case Manager serves as the primary clinical
lead for an assigned patient census, coordinating care across
disciplines while providing skilled nursing services, symptom
management, and emotional support to patients and families. Key
Responsibilities Manage and coordinate hospice care for an assigned
patient caseload Perform comprehensive assessments and develop
individualized plans of care Collaborate closely with physicians,
social workers, aides, chaplains, and other team members Provide
symptom management, medication oversight, and patient/family
education Ensure accurate, timely clinical documentation and
regulatory compliance Participate in interdisciplinary team
meetings and on-call rotation as scheduled Qualifications Current
Utah RN license in good standing Hospice or home health experience
strongly preferred Strong clinical judgment, organization, and
communication skills Ability to work independently while
functioning as part of a collaborative team Compassionate,
