Description: Job Title: Community Health WorkerReports to: Site
ManagerSalary: Starting at $16.40/hr (salary commensurate with
experience and relevant certifications)Job Summary: -- Responsible
for helping patients navigate and access community services, and
work closely with medical and mental health providers and case
managers to ensure patients have comprehensive, coordinated, and
integrated care. -- Obtain and follow-up on patient data for
value-based care processes. Job Qualifications: -- Bilingual
(Spanish/English) required.-- Licenses/Certifications/Requirements:
o High School Diploma or GEDo CHW or Care management certification
preferredo Previous CHW, Care Coordinator or MA experience
preferredo Current Basic Life Support (CPR) certification required
within 6 months of hireo Proof of current immunizations will be
requested upon hire-- Experience in hospital or clinical setting
preferred.-- Excellent interpersonal communication and customer
service skills.-- Ability to identify, prioritize and complete
tasks with limited supervision.-- Reliable transportation. Limited
travel to other sites for cross-coverage is possible
